Lead Registered Nurse (Hybrid Remote-Bronx, New York) - 19082

Maximus is currently seeking a Lead Registered Nurse to provide outstanding home assessment services to vulnerable populations and meet position qualifications to support the New York Independant Assessor Program (NYIAP). The NYIA Program serves the State of New York by conducting a UAS assessment to determine eligibility for community-based long-term services. In this role, the nurse will have the opportunity to conduct home assessments and develop leadership skills by supporting the Regional Nurse Manager and a team of Nurse Assessors.  

 *This position is hybrid and will support the Bronx area in New York City. The position requires attending and conducting assessments in consumer homes and through a telehealth modality.

- Responsible for conducting clinical appointments and precepting new clinicians
- Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments
- Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes
- Work collaboratively with manager to identify areas for improvement with process and quality of care
- Suggest training needs for assigned unit staff
- Provide technical assistance to assigned unit staff
- Support Company and project initiatives through various methods including messaging to the unit, leading assigned projects, training staff, participating in work groups, etc.
- Participate in performance, improvement, and quality assurance activities and related meetings
- Provide clinical and educational support to meet program goals
- Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template for this position<br />- Perform initial assessments of consumers that includes evaluation of health status, functional status, strengths, care needs, and preferences.

- Review consumer medical documentation and/or health referral forms, as relevant to the case.
- Enter assessment data into the UAS-NY online application and transmit, as required.
- Code the UAS assessment correctly based on training and guidelines in the UAS-NY Community Health Assessment Reference Manual.
- Document outcomes of calls to consumers and scheduled appointments.
- Maintain current knowledge of regulatory and project changes pertaining to quality and validity of assessments.
- Enhance communication and collaborative relationships with leadership and interdisciplinary care team members to facilitate receipt of services.
- Review non-qualified assessments with the Clinical Quality Assurance Department.
- Maintain a comprehensive working knowledge of community resources and network services for the target population.
- Perform other duties as may be assigned by the Regional Nurse Manager or other project management<br />Minimum Requirements:
- Current Registered Nurse (RN) license valid in the state of practice is required
- Two (2) or more years practicing experience as a Registered Nurse
- At least one (1) year of experience in training, precepting, acting as a clinical advisor, providing unit support, working in the field independently/successfully
- Must have active ID Number or be willing to obtain
- Preferred work experience with the adult/geriatric populations; home care experience, experience working with individuals with physical or developmental disabilities
- Required ability to travel throughout assigned region, up to 60%<br />-RN license in the state of New York 
-2+ years RN experience in-home care
-Prior experience using the UAS-NY Community assessment tool, OASIS or MDS
-Ability to travel locally up to 50%
-Reliable form of transportation, may include public transportation services

Preferred Skills: 
- Experience conducting assessments in UAS (role 40 or 45)
- Experience with the geriatric population, chronic diseases, especially in the home setting

 Home Office Requirements:-Candidates must have their own cell phone
