Care Management Internship

Overview:
The Care Management Internship at Thrive West Central offers students hands-on experience in ethical and professional practices, human rights advocacy, policy engagement, and research-informed care management. Interns will develop practical skills in social service and healthcare management while collaborating closely with the Aging Division's Leadership Team.

Supervision and Mentorship:
The intern will be directly supervised and mentored by the Aging Quality and Training Manager or another senior member of the Aging Division. Weekly one-on-one check-ins will provide guidance, feedback, and opportunities for professional growth. The intern will also have opportunities to shadow Aging Team members and participate in team meetings to gain a deeper understanding of care management practices.

Learning Goals:
The internship is designed to help students develop key skills and knowledge, such as:

  • Understand the fundamentals of case management and care planning.
  • Develop practical knowledge of state and federal healthcare and social service programs.
  • Gain experience in professional documentation and compliance standards.
  • Strengthening communication and interpersonal skills through client interactions.
  • Learn problem-solving techniques in addressing client needs and service gaps.
  • Develop organizational and data management skills relevant to care management.
  • Gain firsthand experience in advocating for community engagement and service accessibility.

Qualifications:
  • Current enrollment in Social Work, Psychology, Nursing, Public Health, or a related field.
  • Strong interpersonal and communication skills.
  • Ability to work independently and collaboratively.
  • Basic proficiency in Microsoft Office and data entry systems.
  • Empathy and cultural competence in working with diverse populations.
  • A valid driver's license and reliable transportation are a must.
